Practice Policies
Credit Card on File Policy
All patients are required to provide a credit card to be kept securely on file. Details are listed under Practice Policies.
Cancellation Policy
We kindly require a 24-hour notice for cancellations. Same day cancellations and/or missed appointment fees are $50-$100 and based on the length of the appointment. Please contact us prior to 24 hours at (847) 272-4433 or staff@dpns.net to avoid the fee. Otherwise you will be expected to pay the fee before scheduling your next appointment.

Please note we have seen an increase in policies that limit members to a narrower network of providers. We highly recommend you contact your insurance company to verify that DPNS is in-network with your specific coverage prior to scheduling. When speaking with your insurance company, keep in mind that our office is not affiliated with NorthShore University Health Systems.
Our office does NOT participate with any medical assistance policies such as Medicaid plans. In addition, all HMOs policies, except Northshore/Endeavor BCBS HMO site 447, will require a referral from your primary care physician with an authorization number from insurance.
Prescription Refills
To request a prescription refill, please contact your pharmacy and ask for a refill request to be faxed to (847) 272-4434 or sent electronically.
